For a bug, these four make it fixable without a second round trip:

1. The page, and what you pressed.
2. What happened, and what you expected instead.
3. Phone or computer, and which browser.
4. Roughly when. A time helps me find it in the logs.

Never send a password, an invite code, or anything from a Lovense token. Nobody here will ask you for them, and this form is not the place for anything you would not want sitting in a database.
